  "notes": "This is the 7th quarterly HIV Program report after

implementation of the 2011 Integrated Clinical\r\nHIV Guidelines in 
            
27
July 2011. A summary of the key achievements between January and March 
            
28
2013\r\nis provided below:\r\n\r\nScale\u2010up of integrated HIV 
            
29
services had reached the following number of sites:\r\n\r\n     825 
            
30
static (607 within and 218 outside of health facilities) and 534 
            
31
outreach HTC sites\r\n     \r\n     655 (static) ART sites\r\n     
            
32
\r\n     580 PMTCT sites (Option B+)\r\n     \r\n     602 Pre\u2010ART 
            
33
sites\r\n     \r\n     590 sites with HIV\u2010exposed child 
            
34
follow\u2010up\r\n\r\n460,559 persons were tested and counselled for 
            
35
HIV and 41,321 (9%) were HIV positive;\r\n170,334 (37%) people tested 
            
36
for the first time.\r\n \r\n127,167 (78%) of 163,522 women at ANC had 
            
37
their HIV status ascertained; 10,282 (8%) of\r\nthese were HIV 
            
38
positive. 102,506 (89%) of 114,376 women at maternity had their HIV 
            
39
status\r\nascertained; 8,306 (8%) of these were HIV 
            
40
positive.\r\n\r\n26,881 patients started ART during this quarter; this 
            
41
is an 11% increase from the previous\r\nquarter (24,168), probably 
            
42
related to the improved management of HIV test kit 
            
43
supplies.\r\n\r\n422,866 patients were alive and on ART by end of 
            
44
March 2013; 84,499 (22%) of 390,576 on\r\nfirst line adult regimens 
            
45
were on ART regimen 5A (tenofovir / lamivudine / 
            
46
efavirenz).\r\n\r\n81% of adults and 78% of children were retained 
            
47
alive on ART at 12 months after ART\r\ninitiation.\r\n\r\nA total of 
            
48
11,120 HIV positive pregnant women were on ART: 4,134 (37%) of these 
            
49
were\r\nalready on ART when getting pregnant and 6,986 (63%) started 
            
50
ART during\r\npregnancy/delivery. 6,773 (97%) of pregnant women 
            
51
started ART due to Option B+ (in WHO\r\nclinical stage 1 or 2) and 213 
            
52
(3%) due to a low CD4 count and/or WHO clinical stage 3 or 
            
53
4.\r\n\r\nAn additional 2,279 breastfeeding women started ART due to 
            
54
Option B+ (in WHO clinical\r\nstage 1 or 2).\r\n\r\n6,695 (77%) of 
            
55
8,648 women started under Option B+ were retained at 6 months after 
            
56
ART\r\ninitiation (6\u2010month retention outcomes were not available 
            
57
for some women, mainly at sites\r\nwith electronic data 
            
58
systems).\r\n\r\n7,740 (8%) of infants discharged alive from maternity 
            
59
were known to be HIV exposed, 6,975\r\n(90%) of these received ARV 
            
60
prophylaxis. 5,096 (73%) were enrolled in exposed child 
            
61
followup\r\nbefore age 2 months.\r\n\r\nA total of 10,319 HIV exposed 
            
62
children and 9,510 pre\u2010ART patients were enrolled for 
            
63
followup\r\nin HIV Care Clinics (HCC) during this quarter.\r\n\r\n195 
            
64
medical staff were newly trained and certified in PMTCT/ART and an 
            
65
additional 23\r\nmedical records clerks were trained in M&E for the 
            
66
PMTCT/ART program this quarter. This\r\nbrings the total number 
            
67
trained in the 2011 guidelines to 5,711.\r\n\r\n513 HTC counsellors 
            
68
and 1,239 HSAs at 152 facilities received mentoring for collection 
            
69
of\r\ndried blood samples for Early Infant Diagnosis and documentation 
            
70
of results.\r\n\r\nIn preparation for a nationwide retraining of all 
            
71
HTC providers, 20 Master trainers were\r\ntrained in the HTC Skills 
            
72
Intensive Training curriculum, who in turn trained 197 
            
73
trainers.\r\n\r\n418 health workers and 182 support staff at 70 
            
74
facilities in the SE Zone received clinical\r\nmentoring in provision 
            
75
of PMTCT/ART. 45 of these health workers were in the 
            
76
intensive\r\nphase, 223 in the continuation phase and 150 graduated 
            
77
from the mentoring program this\r\nquarter.",

in 2001, initially as a unit in the Department of Clinical Services,

to coordinate the biomedical HIV Program in Malawi. The department now

sexually transmitted infections (STIs), prevention of mother to child
