  "notes": "By way of reducing the incidence of HIV and other STIs,

the Fourth National Health Plan (1999 \u2013 2004) identifies

\u201cadopting syndromic management of STIs in all health centres, 
            
28
with appropriate training\u201d and \u201cintegrating STI syndromic 
            
29
management in training curricula of medical doctors, nurses, medical 
            
30
assistants and clinical officers\u201d as two of the main activities 
            
31
towards this aim. The National Reproductive Health Strategy 1999 
            
32
\u2013 2004 and the Malawi National HIV/AIDS Strategic framework 2000 
            
33
\u2013 2004 further endorse the importance of improving the management 
            
34
of STIs both in their own right, and in the prevention of transmission 
            
35
of HIV.\r\nThe Malawi Government adopted the Syndromic Management 
            
36
Approach in STIs in 1992, a policy decision endorsed by the results of 
            
37
a STI care management survey by Chilongozi et al (1996). The Ministry 
            
38
of Health was assisted by JSI-STAFH project in the implementation in 5 
            
39
pilot sites. Based on the results of the JSI-STAFH project evaluation 
            
40
in 1997, service providers and trainers handbooks were revised and the 
            
41
duration of the training was extended to two weeks.\r\nThis edition of 
            
42
the service providers handbook updates the earlier versions based on 
            
43
the review and revision of the SMA Flowcharts by the STI taskforce in 
            
44
2002. The taskforce aimed to make the flowcharts more user friendly, 
            
45
which has resulted in merging the 4 separate flowcharts for 
            
46
Genitourinary symptoms in women into one flowchart named Abnormal 
            
47
Vaginal Discharge. Two new flowcharts have been introduced, one for 
            
48
recurrent urethral discharge and one for neonatal conjunctivitis. All 
            
49
antibiotic regimens recommended in the flowcharts are based on 
            
50
previous guidelines, WHO recommendations and clinical efficacy studies 
            
51
carried out in Lilongwe by the University of North Carolina Project in 
            
52
